Home health care services can help people receive certain skilled clinical and rehabilitation services where they live. For families, the most important first step is understanding which needs are medical, which are related to daily living, and how the care plan brings different professionals together.

Medicare describes home health as health care provided in the home for illness or injury and lists skilled nursing, therapy, medical social services, and certain aide services among covered benefits for eligible patients.

How home health differs from non-medical home care

Home health is generally clinical and tied to medical or rehabilitation needs. Non-medical home care can support bathing, dressing, grooming, meals, companionship, errands, household routines, and other daily activities. The National Institute on Aging describes both health-related and personal support as important parts of helping people remain at home.

How a skilled plan of care works

For Medicare-covered home health, a qualified provider must establish and periodically review a plan of care. CMS explains that the plan should reflect the patient’s condition and skilled nursing or therapy needs. Families should know who is responsible for each service, what goals are being addressed, and how progress or changes will be communicated.

Coverage is not the same as need

A person may benefit from help at home even when a particular service is not covered by Medicare or another payer. Medicare’s home health benefit has specific eligibility requirements and generally does not pay for 24-hour care at home or personal care when that is the only care needed. Always verify benefits for the individual plan and situation.
